Care Plans and Team Care arrangements are conducted at Hill Medical Services for all patients with a chronic illness.
Our doctors and nurses manage patients with a range of chronic conditions including but not limited to Osteoarthritis, Asthma, Coronary Heart Disease, Chronic Obstructive Pulmonary disease, Obesity, Diabetes and renal impairment and lung disease. Hill Medical also has a range of allied health professionals on site that can assist you to maximise your health and an excellent network of specialists and allied health professionals.
They provide a support system and liaise with allied health professionals in order to provide optimum health care. Reviews of these care plans will be done 3-6 monthly depending on the chronic illness and the goals to be achieved.
5 allied health visits will be allocated to the patient per calendar year under the recommendation of the GP. These visits may be bulk-billed by the allied health professionals here at Hill Medical Services.
Dr Tereza Ghaly, Dr Mary-Ann Boulos and our other female GPs are an experienced GPs who provide shared antenatal care with Monash Health.
This allows pregnant women to have the opportunity to see their own GP who is aware of their history and also be under the care of the hospital.
Please bring your antenatal record to each visit, so that your GP is able to complete it.
6 weeks post the birth of your child, an appointment is required for yourself and your baby( long appointment needed). Please bring the health and development book (Green Book) along to your appointment as your child may be eligible to have their vaccinations done on that day (baby must be at least 6 weeks old to receive vaccine).
Please inform staff when booking any of these appointments.
Osteoarthritis and joint degeneration is very common in our ageing society and at Hill Medical Services we provide allied health services such as physiotherapy, chiropractric services and exercise physiology on site. Sports injuries can be treated by our allied health team as well. We all aim to improve your health and well-being.
Our Doctors and Nurses will be able to help by setting up a team care arrangement if you suffer from a chronic illness. By setting up this plan you will be eligible to visit our services(bulk-billed under medicare) for 5 combined allied health visits per calendar year-as decided by your doctor.
If further intervention is required for injuries incurred then our doctors are able to send you to imaging services and specialists that will be of further assistance.
No requests for these will be taken over the phone. Appointments can usually be found within 1-2 days. Please make sure you make an appointment before your medication finishes.
The philosophy of this practice is ‘excellence in health care’. We aim to provide comprehensive and thoughtful medical care to families.
Doctors will try to return your phone calls if you leave a message. Urgent matters are dealt with promptly. Our
practice has a computerised recall system.
Booking a long appointment. If you want an insurance medical, review of a complex health problem, counselling for emotional difficulties, or have a complaint, please make sure you book a longer appointment. Reports, forms and applications etc will attract an out of pocket charge- usually payable before completion. They usually will not be able to be completed at the appointment time depending on the nature of the request.
Please bring relevant letters and test results from other doctors.
Please notify us if you are unable to attend an appointment 24 hours in advance, otherwise an Administration fee will be charged. If you have received a courtesy SMS reminder for your appointment- please reply YES to confirm and NO to cancel. We may then be able to offer your appoitment time to another patient who needs it.
If more than one person from your family wishes to see the doctor at the same time, please ensure a separate appointment is made for each family member.
Home visits. Home visits and nursing home/hospital visits may be arranged for those unable to attend the practice if current patient of the clinic and with prior arrangement with your usual GP.
Despite our best intentions, we sometimes run late! This is because someone has needed unexpected urgent attention. Be assured when it comes to your turn the doctor will give your problem the time it deserves. Thank you for your consideration.

To ensure patients who receive care from the Practice are comfortable in entrusting their health information to the Practice. This policy provides information to patients as to how their personal information (which includes their health information) is collected and used within the Practice, and the circumstances in which we may disclose it to third parties.

1. Practice staff collects patients’ personal and demographic information via registration when patients present to the Practice for the first time. Patients are encouraged to pay attention to the collection statement attached to/within the form and information about the management of collected information and patient privacy.
2. During the course of providing medical services, the Practice’s healthcare practitioners will consequently collect further personal information.
3. Personal information may also be collected from the patient’s guardian or responsible person (where practicable and necessary), or from any other involved healthcare specialists.
The Practice holds all personal information securely, whether in electronic format, in protected information systems or in hard copy format in a secured environment.
Use and disclosure of information
Personal information will only be used for the purpose of providing medical services and for claims and payments, unless otherwise consented to. Some disclosure may occur to third parties engaged by or for the Practice for business purposes, such as accreditation or for the provision of information technology. These third parties are required to comply with this policy. The Practice will inform the patient where there is a statutory requirement to disclose certain personal information (for example, some diseases require mandatory notification).
The Practice will not disclose personal information to any third party other than in the course of providing medical services, without full disclosure to the patient or the recipient, the reason for the information transfer and full consent from the patient. The Practice will not disclose personal information to anyone outside Australia without need and without patient consent.
Exceptions to disclose without patient consent are where the information is:
The Practice will not use any personal information in relation to direct marketing to a patient without that patient’s express consent. Patients may opt-out of direct marketing at any time by notifying the Practice in a letter or email.
The Practice evaluates all unsolicited information it receives to decide if it should be kept, acted on or destroyed.
Access, corrections and privacy concerns
The Practice acknowledges patients may request access to their medical records. Patients are encouraged to make this request in writing, and the Practice will respond within a reasonable time.
The Practice will take reasonable steps to correct personal information where it is satisfied they are not accurate or up to date. From time to time, the Practice will ask patients to verify the personal information held by the Practice is correct and up to date. Patients may also request the Practice corrects or updates their information, and patients should make such requests in writing.
The Practice takes complaints and concerns about the privacy of patients’ personal information seriously.
Patients should express any privacy concerns in writing. The Practice will then attempt to resolve it in accordance with its complaint resolution procedure.

At Hill Medical Services we have accreidtation to provide Yellow Fever Vaccinations. A long appointment is required and you must bring your passport to your appointment so the nurse is able to document your vaccination.
We also provide all other types of travel vaccinations, please inform staff when you are making your appointment that it is for travel vaccines and all vaccines must be paid on the day of the appointment. (cash, EFTPOS accepted)
